**Youth-Designed Vaccine Clinic**

WHERE
Thorncliffe Park Community Hub at East York Town Centre (45 Overlea Boulevard, Toronto, ON M4H 1C3) (in the parking lot)

WHO
FIRST DOSE ELIGIBILITY:
- Age 12 or older on day of vaccination;
- Live/work/attend school in any "M" postal code
SECOND DOSE ELIGIBILITY:
- Age 12 or older on day of vaccination;
- Live/work/attend school in any "M" postal code
AND
- Received your first dose of Pfizer at least 21 days ago; or
- Received your first dose of Moderna at least 28 days ago; or
- Received your first dose of AstraZeneca at least 56 days ago
WHEN
12 p.m. to 7 p.m. or while vaccine supply lasts.
HOW
Drop-ins welcome. No appointment is required. Pfizer will be administered. Moderna will also be available to those who request it.
Youth ages 12 to 26 who receive their first or second dose at this clinic will also have access to free youth employment services, youth-centered mental health resources, and sports and activities with MLSE LaunchPad. There will also be free ice cream and a DJ!
Plus, all youth who receive their first dose will be entered to win 1 of 10 grants towards any professional or personal skill development opportunity. Youth who bring two friends for their first dose will also be entered to win gift cards!
BRING
Health card is not required if you do not have one. You may bring an ID or document with your name, like a driver's license, passport, utility bill, pay stub, student ID, or report card.
Even if you do not have these documents, we encourage you to attend one of these clinics. Our team will do their best to help get you vaccinated.
Physicians and nurses will be onsite to answer your questions about the vaccine.
